libnice is a good implementation of ICE(internet connectivity establishment) that has a transmitter for farsight. so using farsight and libnice together will allow you to accomplish what you're looking for. as far as writing a program to set up an RTP conference across NAT, i think that's outside the scope of a simple HOWTO.
